How to file a Player Sendoff / Coach Dismissal Report?Printable


Please use the Playing League Match Number on your send off reports. Not the Pensra Match Number.

Always contact your assignor if you have sent off a player or a coach.  ALWAYS


For CCSL Coast Section, CCSL Bay Region and CCSL State Premier

  • From GotSportr website, complete a Send-Off Form->
    • Within 24 hours of the match, fill out this web-based form.
    • Detailed instructions are at CYSA CCSL website.-> Navigate to Referee Procedures or Game Incident Reports.   
    • Retain a copy of the match report. Do not retain the player/teamstaff pass.
    • Any problems contact Dane Denkers        
    • Note that currently (8/9/2022), to complete a SendOff report, you must register with the GotSport website
  • Contact your Assignor
    • Within 24 hours of the match, let the Assignor of the match know about the incident. Open communication and no surprises are the goal here. Go to Referee:Inquiry and find the match in question. Drill down to the match detail by clicking on the hyperlinked match number to see the Assignor.

For NorCal Premier, NorCal State Cup, NorCal Regional and other NorCal programs

  • From norcalreferees.com website, complete a Send-Off Form->
    Within 24 hours of the match, fill out this web-based form, hit submit. The website validates the data submitted by the referee. The referee and appropriate authority, if available, receive an email with the official NorCal Send Off Report attached as a PDF, but pre-filled with all the data submitted. The email will have additional instructions. The attached PDF can be saved, printed and mailed with the player's or coach's card to the appropriate authority. Any problems contact Aidon Jennery

  • From sfyouthsoccer.com-> website, complete a Send-Off Form
    Within 24 hours of the match, fill out this web-based pdf form. Copy it to your computer and email it to the SF Youth Soccer and League Referee Assignor. (Details on the SFYSL website.) Any problems contact David Reardon.


For AYSO Area 2N

  • Referee complete a "24 hour Incident Report" available at www.ayso2n.org.-> It is a 'PDF' document that must be downloaded, completed and saved, then e-mailed to the appropriate League officials.
  • Referee retains the Coach or Player Pass.
  • Mail the 24 hour report to Clayton Wishoff, 367 Menhaden Court, Foster City, CA 94404.
  • Mail the retained pass(es) to Charlie Drechsler, 203 Ottawa Street, San Mateo, CA, 94401.
